The paper chase
The PDF that never comes back
You email a form. It needs a printer, a pen, and a free evening. It sits on a counter for three days — then the client gets busy, and the lead goes quiet.
For independent insurance producers · Early access
AllOneForm helps independent producers capture a new prospect's details in minutes instead of days of calls, texts, and paper forms. Your client answers once, in a tap-first intake. You get one structured record, ready to review and take into your rater.
Early access opens first to a small group of independent producers. The walkthrough is where you watch it work on a real client path — and decide with proof, not promises.

The leak in every agency
Not because clients don't want the quote — because the way they're asked for details fights them. Every unfinished form is a household another agency gets to quote first.
The paper chase
You email a form. It needs a printer, a pen, and a free evening. It sits on a counter for three days — then the client gets busy, and the lead goes quiet.
The phone tag
Drivers on a voicemail. The mortgagee in a text thread. A VIN in a photo you have to squint at. You spend your selling hours collecting instead of quoting.
The retyping
Whatever you finally collect gets keyed into your rater by hand — slowly, and with every typo a chance to quote the wrong thing.
AllOneForm replaces all three with one link your client finishes on their phone — and a record that lands ready to quote. See how it works
What it is
Most intake loses time in the gaps. A client starts, gets busy, and the details sit in a voicemail or a text thread while another agency moves ahead. AllOneForm closes those gaps with automation, so more of the people who start with you can finish with you.
The intake is tap-first, with one thing per screen and taps instead of typing where a tap will do. You take it from there.
Progress saves as your client goes, and each client gets a personal resume link to pick up right where they left off. If a client calls instead, you can open that same form in agent assist mode and finish it with them on the phone.
Household drivers, vehicles, home details, and mortgagee information arrive as structured data, with anything a client is unsure of flagged for your check. The record is ready to take into your rater instead of sitting in notes you have to retype.
How it works
Your intake lives at its own AllOneForm address, arranged during setup. Put it on a business card, a QR code, or a text message.
Clients with current policy pages (declarations pages) can use the dec page fast lane. They upload their pages and answer only the gaps. Clients without pages take the guided full form.
Everything arrives in your producer backend, organized and readable, with a short list of anything that still needs your eye. You take the structured record into your rater (EZLynx, or the rater you already use) and start the quote. Your agency system stays your system of record.
Every answer organized on one sheet, in a consistent order, with a copy summary and a print view — worked straight into your rater. Nothing to hunt for across texts, voicemails, and sticky notes.
When a client who has given consent, and has not opted out, stops partway, AllOneForm prepares a follow-up for that intake automatically — the chasing happens without you working a call list. It waits for your review. Nothing sends itself, and live sending turns on at launch, only for clients who consented.
Inside the intake
One job — get a complete, quotable record out of a busy client — and every part of the form is aimed at it.
One question per screen, big tap targets, dropdowns and bubbles instead of blank boxes. Built for the phone in your client's hand.
Clients with current policy pages upload them and answer only the gaps. The fastest path from referral to a quotable record.
Every start past contact details saves itself. A personal resume link reopens the same form, on any device, right where they stopped.
If a client would rather talk, open their exact intake and finish it with them on the call — on the same record they started.
The lender and its exact address are collected in the form, with a “not sure — agent will verify” escape instead of a guess.
Your agency's name, logo, and colors lead every client-facing page. Clients experience your agency's intake — AllOneForm stays in the background.
The producer backend
Intakes land in one pipeline: new, in progress, complete, and the short list that needs your eye. Open a record and everything the client gave you is on one sheet.
Who it is for
AllOneForm starts with personal home and auto intake, the front door of an independent agency. You buy back the hours you now spend chasing details by phone and text, and it helps you keep prospects who might otherwise drift away mid-form. Your name and your agency lead every client facing page. AllOneForm works in the background and stays out of the relationship.
That is the whole product, on purpose. AllOneForm is a client intake form, and our full focus is making it an intake designed to be finished: one thing per screen, taps instead of typing. When an intake is complete, the finished record hands off to your rater, and your agency system stays your system of record for the policy. We get the client in, structured and ready for you to quote from. Your existing tools take it from there.
AllOneForm was built by a working independent producer, for his own agency first, and for producers who would rather be quoting than chasing paperwork.
Your book, your data
You are trusting AllOneForm with the front door of your client relationships. That trust is the product as much as the form is.
Every producer signs in to their own agency's intakes and nothing else. Your client list is never shared with, or visible to, another agency.
Your clients' details are never sold, and they are never used to market anything to your clients. Follow ups about an intake will go out only with that client's consent, and prepared follow ups wait for your review.
Your agency's name, logo, and colors lead every client facing page, with your name, title, and license number the way you present them. Clients experience your agency's intake. AllOneForm works in the background and stays out of the relationship.
Client details move over an encrypted connection and are stored with each agency's data kept separate from every other agency's. Producers reach them only by signing in to their own agency. Sensitive numbers, like a Social Security number, are optional in the intake and are shown masked, last four digits only, on producer screens.
How the forms on this site handle your details is written out in our privacy note.
Common questions
No. AllOneForm is the intake in front of them. The finished record goes into your rater (EZLynx, or the rater you already use) to start the quote, and your agency system stays your system of record.
No. They open your intake link and start. There is no account and no password, and a personal resume link lets them pick up where they left off.
